This position provides intake, evaluation, referral, and monitoring services for children birth to three years of age who are being considered for or enrolled in the Early Intervention Program.

Essential duties include conducting intakes, coordinating evaluations, developing IFSPs, monitoring services, and maintaining Pelican records, while collaborating with families and external agencies to ensure timely, appropriate services.

Qualificações

• Bachelor's degree with a social welfare major.
• Must obtain clearances from PA State Police, FBI, PA Child Abuse, and OIG. Maintain clearances.
• Six months of experience as a Human Services Case Manager or equivalent.
• Knowledge of early intervention regulations and related laws.
• Strong communication, organization and confidentiality skills.

Responsabilidades

• Conduct intakes with families and schedule evaluations for eligibility.
• Develop and coordinate Individualized Family Services Plans (IFSP).
• Monitor services provided to families.
• Maintain records and documentation in Pelican system.
• Interact with families, agencies, and providers regarding duties.
• Advise parents of their rights and safeguards for EI services.
• Serve as advocate for children and parents.
• Promote safety in the family’s environment.
• Attend meetings, trainings, and required in-services.
• Travel frequently to agencies and homes; some evenings required.
