Hand In Hand Development, Inc., CMA is a NYS DOH approved Early Intervention Agency that provides evaluations, service coordination, and therapeutic services for children birth to 3 in all 5 boroughs, upstate, and Long Island.
Hand In Hand is also an approved Care Management Agency (CMA) under the lead health home of the Collaborative for Children and Families (CCF), providing comprehensive care management, health promotion, transitional care, and family support for children and young adults from birth to age 21. It is the largest CMA in New York and is recognized as a leader and innovator in the provision of care management services.
