Job summary

Centerville Post Acute is seeking a licensed Physical Therapist Assistant (PTA) to join our rehabilitation team. You will work under the supervision of a Physical Therapist to help residents regain mobility and independence.

The PTA will provide skilled treatments, educate residents and families, document progress in the EMR, and collaborate with a multidisciplinary team in a post-acute care setting.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Physical Therapist Assistant license in the State of Ohio.
• CPR certification required.
• Minimum of 2 years of Physical Therapist Assistant experience preferred, post-acute or skilled nursing preferred.

Responsibilities

• Administer physical therapy treatments and exercises under the supervising PT.
• Improve residents' mobility, strength, balance, coordination and independence.
• Monitor progress and report changes to the supervising PT.
• Document treatments and progress in the EMR.
• Educate residents and families on exercises and safety.
• Assist with transfers, gait training and functional mobility exercises.
• Participate in interdisciplinary care planning meetings.
• Maintain clean and safe treatment areas and equipment.
• Follow facility policies and Medicare documentation requirements.
• Collaborate with nursing, OT, SLP, physicians and other team members.
