Talented Medical Solutions in Twin Falls, Idaho is seeking a compassionate Physical Therapist to provide high-quality, patient-centered rehabilitation in a healthcare setting.

The PT will evaluate patients, develop individualized plans, and collaborate with physicians and nurses to ensure safe, evidence-based care across acute, outpatient, inpatient rehab, SNF, and home health settings.

Qualifications

• Active PT license in the state of practice.
• BLS/CPR certification per facility policy.
• DPT degree or equivalent as required by state regulations.
• Experience in acute care, outpatient, inpatient rehab, SNF, home health, or specialty settings preferred.
• Strong clinical assessment, documentation, and patient education skills.
• Ability to work independently and collaboratively with an interdisciplinary team.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and physician referrals.
• Provide therapeutic interventions to improve mobility, strength, balance, and functional independence.
• Monitor progress, modify treatment plans as appropriate, and document evaluations, treatments, and outcomes.
• Educate patients, families, and caregivers on treatment plans and home exercise programs.
• Collaborate with physicians and interdisciplinary teams to coordinate patient care.
• Maintain accurate clinical documentation in line with policies and regulatory requirements.
• Ensure patient safety, infection prevention, and quality assurance standards.
• Treat patients across assigned care settings and age groups as required.
