Job summary

Talented Medical Solutions is seeking a compassionate Physical Therapist (PT) to deliver high-quality rehabilitation services. The PT will evaluate patients, design individualized plans, apply therapeutic interventions, and monitor progress across care settings.

The role requires active PT licensure in the state, a DPT or equivalent, and strong clinical, communication, and documentation skills to support evidence-based patient care.

Qualifications

• Active Physical Therapist license in the state of practice.
• Current Basic Life Support (BLS/CPR) certification preferred or required by facility policy.
• Doctor of Physical Therapy (DPT) degree or equivalent as required by state regulations.
• Experience in acute care, outpatient, inpatient rehabilitation, skilled nursing, home health, or specialty settings preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and physician referrals.
• Provide therapeutic interventions to improve mobility, strength, balance, flexibility, coordination, pain management, and functional independence.
• Monitor patient progress, modify treatment plans as appropriate, and document evaluations, treatments, and outcomes.
• Educate patients, families, and caregivers regarding treatment plans, home exercise programs, injury prevention, and discharge recommendations.
• Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care and achieve rehabilitation outcomes.
• Maintain accurate and timely clinical documentation in accordance with facility policies and regulatory requirements.
