Job summary

Talented Medical Solutions in Maryland is seeking a compassionate Physical Therapist (PT) to provide patient-centered rehabilitation across acute and ambulatory settings. You will evaluate patients, develop individualized treatment plans, implement evidence-based interventions, and monitor progress to restore mobility, reduce pain, and improve function.

The role requires active PT licensure in the state, DPT degree, and strong collaboration with physicians, nurses, and interdisciplinary teams.

Qualifications

• DPT degree or equivalent per state requirements.
• Active PT license in the practice state.
• BLS/CPR certification as required by facility policy.

Responsibilities

• Evaluate patients and create individualized treatment plans based on referrals.
• Provide interventions to improve mobility, strength, balance, and independence.
• Monitor progress and document evaluations, treatments, and outcomes.
• Educate patients and families on home exercises and discharge planning.
• Collaborate with physicians and healthcare teams to coordinate care.
• Maintain compliant, accurate clinical documentation per policies.
• Follow patient safety, infection control, and QA standards.
• Treat patients across age groups as required.
• Perform other duties as assigned.
