Qualifications

• Bachelor's degree in Physical Therapy.
• Licensed Physical Therapist (PT) in IL (IDFPR).
• BLS within 30 days - American Heart Association (AHA).
• Attend and satisfactorily complete continuing education for acute stroke care.

Responsibilities

• Demonstrates ability to transfer patients with assistance and uses safe body mechanics.
• Designs and updates plans of care with goals, treatment, outcomes and discharge planning.
• Educates patients, families, caregivers, and other health care providers.
• Evaluates clinical findings to determine PT diagnoses and outcomes of care.
• Performs examinations using reliable methods relevant to the chief complaint, screening, and history.
• Produces documentation per CTSCP67.
• Screens patients to determine therapy needs and effectiveness.
• Supervises PTA and rehab technicians as allowed by regulation.
• Complies with annual education for DNV Hip and Knee Replacement program.
