Job summary

Carle Health is seeking a Physical Therapist to evaluate and treat patients of all ages, adjusting plans of care as needed and serving as a clinical resource for staff and students.

You will oversee patient transfers, educate patients and families, perform examinations, document per policy, supervise PTA and rehab staff, and collaborate with the care team to plan discharge and needed equipment.

Qualifications

• Bachelor's Degree in Physical Therapy.
• Licensed Physical Therapist (PT) with IDFPR.
• Basic Life Support (BLS) within 30 days - American Heart Association.
• Complete required continuing education regarding acute stroke care.

Responsibilities

• Demonstrates ability to perform patient transfers with assistance and uses body mechanics safely.
• Treats patients with dignity and empathy while staying engaged.
• Designs and updates care plans integrating goals, treatment, outcomes, and discharge.
• Educates patients, families, and staff using effective teaching methods.
• Evaluates findings to determine PT diagnoses and outcomes of care.
• Performs examinations with reliable methods based on history and screening.
• Produces documentation per CTSCP67 policy.
• Screens patients to assess therapy needs and effectiveness.
• Uses and supervises support personnel within legal/ethical guidelines.
• Completes annual education for DNV Hip and Knee Replacement requirements if applicable.
• Evaluates/intervenes with functional activities and considers patient/family needs.
• Uses standardized outcomes measures to reflect functional performance.
• Completes documentation per regulatory standards.
• Collaborates with patient and family throughout rehab and provides education.
• Supervises PTA and rehab technicians as required by regulation.
• Communicates performance/status, discharge recommendations, and equipment needs to the medical team.
