Qualifications

• Bachelor's Degree in Physical Therapy required.
• Licensed Physical Therapist (PT) with IDFPR.
• Basic Life Support (BLS) within 30 days - AHA.
• Attend and satisfactorily complete all required continuing education regarding the care of acute stroke patients.

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed from bed, chair and floor, while maintaining safe body mechanics.
• Treats patients with dignity and respect, demonstrating empathy and engagement.
• Designs and adjusts a plan of care with goals, outcomes, and discharge planning.
• Educates patients and families using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes of care.
• Performs examinations using reliable methods.
• Produces documentation per CTSCP67 policy.
• Screens patients to determine need for therapy services.
• Supervises support personnel and provides feedback.
• Completes annual education for DNV Hip and Knee Replacement (if applicable).
• Evaluates and implements functional intervention plans and uses standardized outcomes measures for accurate progress.
