Job summary

The ideal candidate holds a Bachelor's degree in Physical Therapy, is licensed to practice as a PT (Illinois IDFPR or eligible), and has BLS certification.

Qualifications

• Licensed PT in the United States with current credentials and eligibility to practice.
• Bachelor's Degree in Physical Therapy from an accredited program.
• BLS certification to be obtained within 30 days of hire.
• Commitment to ongoing education in acute stroke care and rehabilitation.

Responsibilities

• Designs an appropriate plan of care that integrates goals, treatment, outcomes, and discharge plan.
• Educates patients, families, and staff using effective teaching methods.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
• Performs examination, selecting reliable and valid examination methods relevant to the chief complaint, results of screening, and history.
• Produces documentation in accordance with the Clinical Process Policy (CTSCP67).
• Collaborates with patient, family and significant others throughout rehabilitation and provides education/family training as necessary.
• Supervises PTA and rehabilitation technicians per regulatory guidance.
• Communicates patient performance, status changes, and discharge recommendations to the medical team.

Education

Bachelor's Degree in Physical Therapy

Licensed Physical Therapist (PT) in the United States

BLS Certification (American Heart Association)

Continuing education in acute stroke care
