Job summary

Lexington is seeking a Staff Physical Therapist to evaluate residents, develop treatment plans and supervise PT staff in a facility setting. You will coordinate with physicians and families to ensure goals are met and discharge planning is appropriate.

The role requires a PT license, strong documentation, and ability to educate residents and caregivers on home programs and safety techniques. Training and QA participation are part of the duties.

Qualifications

• Bachelor's degree in Physical Therapy; masters/doctorate preferred.
• Proficient computer skills including Net Health and Point Click Care.
• Licensed Physical Therapist in the state with active license; CE requirements maintained.
• CPR certification, if required by licensure.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and aides in resident care.
• Communicate with physicians, nursing staff, residents, and families about progress and goals.
• Provide in-services and contribute to QA, performance improvement, and rehab program development.
• Maintain timely, accurate, compliant documentation and infection control.
