Job summary

CommonSpirit Health at Home is seeking a compassionate Physical Therapist to provide home-based rehabilitation services, developing personalized plans to improve mobility and daily function for patients in Lexington and surrounding areas.

The role emphasizes one-on-one patient care, interprofessional collaboration, and documentation in compliance with regulations; requires a PT license, bachelor's degree, CPR/BLS, and a valid driver's license, with a preference for 1 year of PT experience.

Qualifications

Bachelor's degree from an accredited school approved by APTA.
Active PT license in the practicing state.
Must obtain and maintain CPR/BLS certification.
One year PT experience preferred but not required.
Current driver's license.
Must comply with applicable laws and regulations.

Responsibilities

Conduct home-based evaluations of patients' physical abilities and mobility.
Implement therapeutic exercises, manual therapy, and modalities to improve function.
Educate patients and families on exercises, safety, and lifestyle changes.
Monitor progress and adjust treatment plans to achieve outcomes.
Maintain accurate patient records in compliance with regulations.
