Job summary

LEXINGTON in is seeking a Full-Time Physical Therapist to evaluate and treat residents, coordinate with families and physicians, and maintain detailed medical records. The role includes training facility staff and supervising PT assistants to ensure high-quality rehabilitation services.

The incumbent will develop treatment plans, educate residents and caregivers, and ensure compliance with infection control, safety, and equipment maintenance protocols.

Qualifications

• Bachelor's degree in Physical Therapy; advanced degrees preferred.
• Licensed in the state with continuing education requirements.
• CPR certification if required by licensure.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and strength.
• Supervise PT assistants and staff in resident care.
• Communicate progress with physicians, nursing staff, residents and families.
• Complete timely, accurate documentation per policies and payer requirements.
