Job summary

Lexington-Post-Acute in Lexington, VA is seeking a Staff Physical Therapist to evaluate residents, develop treatment plans, and coordinate care with physicians, nursing staff, families, and other health team members. You will supervise PT assistants, provide in-services, maintain documentation, and support discharge planning and fall prevention.

A state PT license and ongoing education are required to deliver high-quality rehabilitation services and to participate in interdisciplinary teams.

Qualifications

  • Bachelor’s degree in Physical Therapy; state licensure.
  • Master’s or Doctorate in Physical Therapy preferred.
  • Licensed Physical Therapist in the state and active license.
  • Proficient with Net Health and PointClickCare systems.

Responsibilities

  • Evaluate residents to determine PT needs and rehab potential.
  • Develop and update individualized treatment plans to improve mobility and independence.
  • Supervise PT assistants and aides in resident care.
  • Document services accurately in medical records per policy.
  • Educate residents and families on home exercise programs and safety.
  • Participate in care conferences and discharge planning.
