Job summary

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Licensed Physical Therapist in the state; CPR certification if required.
• Proficiency with Net Health and PointClickCare or similar systems.

Responsibilities

• Evaluate residents to determine PT needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility.
• Supervise PT assistants and trainees in resident care.
• Communicate progress and discharge plans with families and physicians.
• Complete timely documentation in accordance with policies and payer requirements.
• Maintain a safe treatment environment following infection control guidelines.
• Educate residents and families on home exercise and safety.
• Contribute to quality improvement and rehab program development.
