Job summary

Live Well Homecare in Georgia seeks a dedicated Physical Therapist to assess, plan, and implement rehabilitation programs that improve mobility, reduce pain, and prevent deformities in our patients.

You will collaborate with physicians, educate patients and families, document evaluations and progress, uphold professional standards, and participate in ongoing education and onboarding new staff to ensure compassionate, compliant care.

Qualifications

• Graduate from an accredited physical therapy program (Bachelor’s, Master’s, or Doctorate).
• Current Physical Therapist license and BLS certification; BLS Instructor and Provisional certifications preferred.
• Effective communication, customer service focus, teamwork, and basic computer skills.

Responsibilities

• Collaborate with physicians to develop and evaluate treatment plans, goals, and discharge strategies, involving patients and families.
• Implement and adjust therapy plans, educate patients and families, and provide discharge instructions and referrals.
• Document evaluations, treatment sessions, and discharge summaries following departmental protocols.
• Uphold ethical conduct, maintain confidentiality, adhere to policies, communicate effectively, and participate in meetings and training.
• Foster a positive environment, apply AIDET principles, and contribute to service excellence and team collaboration.
• Engage in ongoing education, complete required competencies, and assist in onboarding new staff.
