Job summary

LiveWell Homecare Agency, LLC is seeking a qualified Physical Therapist to assess, plan, and implement rehabilitative programs for clients in a homecare setting. The role aims to improve mobility, reduce pain, and prevent deformities through collaborative treatment planning with physicians, patients, and families.

The PT will conduct patient assessments, develop care plans, implement treatments, educate families, and document evaluations and sessions.

Qualifications

• Graduate from an accredited physical therapy program with a Bachelor's, Master's, or Doctorate in Physical Therapy.
• Hold an active Physical Therapist license and BLS certification; BLS Instructor and Provisional certifications preferred.
• Effective communication, customer service orientation, teamwork, and basic computer skills.
• Commitment to ethical standards, diversity, and patient confidentiality.

Responsibilities

• Conduct patient assessments and develop individualized care plans.
• Implement and adjust treatment activities, providing education and discharge instructions.
• Document evaluations, care plans, and treatment sessions in accordance with departmental guidelines.
• Act as a resource for staff, families, and physicians regarding rehabilitation services and equipment.
• Maintain confidentiality, adhere to policies, and ensure safety protocols are followed.
• Engage in professional development, mandatory training, and participate in team activities.
