Job summary

LiveWell Homecare Agency, LLC is seeking a licensed Physical Therapist to assess, plan, and implement rehabilitation programs aimed at improving mobility and reducing pain for patients receiving in-home care in Roswell, GA.

You will collaborate with physicians, revise goals with patients and families, document evaluations and treatments, and participate in ongoing professional development while upholding infection control and patient confidentiality.

Qualifications

• Doctorate, Masters, or Bachelors in Physical Therapy required.
• Valid PT license and BLS certification required.
• Effective communication, customer service orientation, teamwork and basic computer skills.

Responsibilities

• Implements treatment plans and educates patients and families.
• Collaborates with physicians to assess and adjust goals and discharge plans.
• Maintains accurate documentation of evaluations, treatments and discharge summaries.
• Upholds ethical standards, confidentiality, infection control, and teamwork.
• Engages in ongoing education and maintains licensure and competencies.
