Job summary

Comfort Home Health Agency Inc. is seeking a Physical Therapist to provide skilled therapy in a home-health setting in Florida. You will assess patients, design individualized treatment plans, and guide families on exercises and self-care to promote recovery.

You will collaborate with physicians and other clinicians, document progress, and adjust interventions to maximize function and reduce pain. A commitment to evidence-based practice and patient education is essential.

Responsibilities

• Provide educational information about physical therapy and injury prevention.
• Plan, prepare and carry out individually designed treatment programs.
• Perform an initial exam and determine a diagnosis prior to intervention.
• Evaluate effects of treatment and adjust as needed.
• Administer manual exercises, massage and/or traction to relieve pain and improve strength.
• Instruct patients and families in treatment procedures for home use.
• Confer with patient and medical practitioners to plan and assess the intervention program.
• Review physician's referral and patient records to determine required therapy.
• Record prognosis, treatment, response, and progress in chart or system.
• Discharge patient from physical therapy with follow-up care or referrals.
• Test and measure strength, mobility and function and record data.
• Identify goals, progress and reevaluation plans.
• Explain proposed interventions, risks, benefits and alternatives to patients.
• Refer outside scope when necessary and coordinate care.
• Direct and supervise support personnel and communicate effectively.
• Administer treatments with physical agents and modalities.
• Teach physical therapy students and other health professionals.
• Evaluate and adjust orthotic devices as needed and liaise with orthotists.
• Support research and apply findings to practice.
• Engage with community activities and help influence public policy.
• Construct, maintain and repair medical supportive devices; direct group rehabilitation activities.
