Job summary

Carle Health is seeking a licensed Physical Therapist to evaluate and treat patients of all ages, designing care plans and adjusting them based on patient response. You will educate patients and families and document progress in the therapy EMR, collaborating with a multidisciplinary team.

Requirements include a Bachelor’s Degree in Physical Therapy, current PT licensure, BLS within 30 days, and ongoing CE on acute stroke care. A sign-on bonus and relocation assistance are provided.

Qualifications

• Bachelor’s Degree in Physical Therapy.
• Licensed Physical Therapist (PT) with active licensure.
• Basic Life Support (BLS) within 30 days.
• Continuing education focused on acute stroke care.

Responsibilities

• Performs patient transfers with proper body mechanics.
• Treats patients and communicates with empathy and respect.
• Designs and adjusts a plan of care based on goals and response to treatment.
• Educates patients, families, and caregivers using effective methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes.
• Documents assessments and progress in the therapy EMR.
• Screens patients to determine therapy needs and course of care.
• Collaborates with support personnel and supervises as appropriate.
