Job summary

JAG Physical Therapy in New York is seeking a forward-thinking Physical Therapist to assess patient strengths and weaknesses and develop creative, effective treatment plans. You will maintain confidentiality, support a high-energy environment, and encourage patients to achieve goals through your skills and judgment.

Responsibilities include evaluations, plan of care development, documentation in EMR, and collaboration with the team to optimize patient outcomes.

Qualifications

• Bachelor’s or Master’s Degree required; Doctorate preferred.
• Licensed Physical Therapist in the state practicing in.
• Completed state regulated CEU requirement in NY/NJ.
• Valid CPR License.
• Experience with EMR software preferred.
• Excellent oral, written communication.
• Strong clinical decision-making skills.
• Maintains a positive, professional demeanor.

Responsibilities

• Collects and records patient care data per guidelines and third-party payers; handles initial evaluation, daily notes, reassessments, discharge plans, and team communication.
• Records daily billing following department guidelines for productivity.
• Prepares for evaluations; gathers patient history and diagnoses; researches unfamiliar diagnoses.
• Selects and administers tests and measures; synthesizes results to diagnose and plan care.
• Establishes treatment plans; collaborates with interdisciplinary team and patient support system; reassesses needs.
• Identifies need for consultations or referrals; initiates discharge plans.
• Provides education/training to patient and support system (ADLs/IADLs; rehab equipment).
• Responsible for treatment plans and exercise prescriptions for caseload; safe interventions.
