Job summary

JAG Physical Therapy in New York, NY is seeking a dedicated Physical Therapist to evaluate and treat patients across outpatient settings. You’ll develop treatment plans, document progress, and collaborate with a multidisciplinary team to promote healing and functional independence.

Qualifications

• Licensed Physical Therapist in NY (or practicing state).
• Doctorate degree is preferred.
• Current CPR certification.
• Experience with EMR systems is preferred.

Responsibilities

• Collects and documents patient care data including initial evaluation and progress notes.
• Assesses and formulates discharge plans and goals with the team.
• Completes daily billing and tracks productivity as required.
• Obtains patient history and evaluates diagnoses for plan of care.
• Develops and implements treatment plans and goals with the patient and team.
• Oversees support staff and participates in annual billing and coding seminars.
• Educates patients and families to support rehabilitation and independence.
