Job summary

Carle Health in Illinois is seeking a Licensed Physical Therapist to plan and administer outpatient physical therapy programs. You will develop treatment plans to alleviate pain, restore function, and promote patient health, while serving as a clinical resource for staff and students.

Responsibilities include patient transfers, education, patient evaluation, and collaboration with the medical team, with opportunities to mentor students and participate in program activities.

Qualifications

• Licensed Physical Therapist (PT) in Illinois
• Basic Life Support (BLS) within 30 days (AHA)
• Bachelor's Degree: Physical Therapy (Required)

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed.
• Maintains safe body mechanics and treats patients with dignity and respect.
• Designs and updates plan of care with goals and discharge plan.
• Adjusts care based on patient response and evidence.
• Educates patients and caregivers using effective teaching methods.
• Evaluates clinical findings for PT diagnoses and outcomes.
• Performs examinations with reliable methods.
• Screens patients to determine need for therapy services.
• Monitors care delivered by support staff and provides feedback.
• Communicates with the medical team and documents as required.
• Meets customer service and productivity standards.
• Acts as clinical instructor when assigned.
• Participates in team and department activities.
• Performs reporting and other duties as assigned.
