Job summary

Carle Health is seeking a licensed Physical Therapist in Illinois to plan and administer PT treatment plans that alleviate pain, restore function, and promote overall health. The role serves as a clinical resource and educator for staff and students within a multidisciplinary team.

Responsibilities include designing care plans, monitoring patient progress, and documenting outcomes, while delivering compassionate care and ensuring safety and dignity for every patient.

Qualifications

• Licensed Physical Therapist (PT) in Illinois.
• BLS certification within 30 days.
• Bachelor's Degree: Physical Therapy (Required).
• Experience in outpatient orthopedic settings.

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed from bed, chair and floor.
• Designs an appropriate plan of care that integrates goals, treatment, outcomes, and discharge plan.
• Adjusts the care based on patient response and available evidence.
• Educates patients, family, caregivers, students, and other health care providers using relevant teaching methods.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
• Performs examination and selects reliable examination methods relevant to chief complaint, screening, and history.
• Monitors the care delivered by support personnel and provides regular feedback.
• Communicates with medical team and performs documentation to standards.
