Job summary

Advocate Health Care seeks a Doctorate-educated Physical Therapist to assess and treat patients with musculoskeletal, neuromuscular, and cardiopulmonary conditions. You will design individualized care plans based on evaluations and functional goals using evidence-based techniques.

You will monitor progress, discharge patients with home programs, educate families, document thoroughly, and collaborate with physicians and care teams. Requires IL/WI PT license and BLS.

Qualifications

• Doctorate in Physical Therapy from an accredited program.
• State PT license in IL or WI upon working.
• Basic Life Support (BLS) certification for Healthcare Providers.
• No Experience Required.

Responsibilities

• Establishes a plan of care based on evaluation results and goals.
• Monitors outcomes and adjusts treatment plans as needed.
• Discharges patients with home programs and family education.
• Collaborates with physicians and teammates to ensure optimal care.
• Maintains documentation per department policy and accreditation standards.
• Manages caseload and coordinates coverage as needed.
