You will document activities and health information in the EHR, follow health care plans, and participate in ongoing training and development opportunities. The position is union, non-exempt, overtime-eligible with a 40-hour weekly schedule.

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver's license with the ability to transport individuals in agency vehicles.
• Ability to complete required background checks, fingerprinting, and pre-employment health screenings.

Responsibilities

• Assist with daily living activities including personal care and hygiene.
• Monitor health information and document vitals as required.
• Transport individuals to appointments and community activities.
• Prepare meals and assist during mealtimes per dietary needs.
• Provide mobility assistance using approved techniques and equipment.
• Document activity, health, and medication records in the EHR.
• Follow emergency procedures and safety protocols when needed.
• Communicate effectively with team, individuals served, and families.
