Job summary

Oak Hill in Wethersfield, CT is looking for a Direct Support Professional for the day shift on a part-time basis (16 hours weekly, weekends 7am-3pm). Compensation starts at $20.39/hour with differential pay available.

You will support individuals with intellectual and developmental disabilities in a residential setting, assist with daily living, health monitoring, medication administration, mobility, meals, and transportation, while documenting care in the EHR and following safety and regulatory

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver's license with the ability to transport individuals in agency vehicles.
• Ability to complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment in accordance with Oak Hill policies.
• Commitment to continuity of care, including an occasional need to stay beyond a scheduled shift until coverage is in place.

Responsibilities

• Support daily living activities to promote independence.
• Assist with personal care, grooming, bathing, dressing, and hygiene needs.
• Providing continence care as needed.
• Encourage independence and skill development.
• Monitor and document health-related information and vital signs when required.
• Administer medications and maintain accurate medication records as trained.
• Follow individualized health care plans and support overall well-being.
• Transport individuals to appointments and activities using agency vehicles.
• Prepare meals and snacks according to menus and dietary requirements.
• Assist with mobility and transfers using approved techniques and equipment.
• Document daily activities and health information in the EHR.
• Comply with Oak Hill, DDS, DPH, HIPAA and regulatory requirements.
• Respond to emergencies and behavioral needs per procedures; apply first aid/CPR when required.
