Job summary

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver's license with ability to transport individuals.
• Ability to complete required background checks and health screenings.

Responsibilities

• Support individuals in daily living, personal care, grooming, bathing and hygiene needs as per care plans.
• Assist with continence care.
• Monitor health information and document vital signs as required.
• Administer medications per training and maintain records.
• Transport individuals to appointments and community activities.
• Prepare meals and provide mealtime support.
• Assist with mobility and transfers using approved techniques.
