Qualifications

• Must be at least 18 years old.
• High school diploma or GED.
• Valid driver's license with the ability to transport individuals in agency vehicles.
• Ability to complete required background checks and health screenings.
• Ability to complete required trainings and certifications.

Responsibilities

• Support daily living activities including personal care, grooming, and hygiene.
• Monitor health information and document vital signs when required.
• Administer medications and maintain medication records per directions.
• Transport individuals to appointments and activities using agency vehicles.
• Prepare meals and assist with mealtime according to dietary needs.
• Assist with mobility and transfers using approved techniques and equipment.
• Document daily activities and health information in the EHR.
• Follow emergency procedures and safety protocols; perform First Aid/CPR when needed.
• Collaborate with team, attend meetings, and participate in trainings.
