Oak Hill is hiring a Direct Support Professional in Southington, CT. The DSP will provide person-centered support in a residential setting, assisting with daily living, health monitoring, nutrition, and community inclusion. This role requires compassion, reliability, and adherence to safety and regulatory standards.

Location: Southington, CT; Schedule: Mon-Fri, 7:00 AM – 3:00 PM; 40 hours/week. Base pay $20.39/hour with differential pay for specific schedules.

Qualifications

• Must be at least 18 years old.
• High school diploma or GED.
• Valid driver's license to transport individuals.
• Ability to complete required background checks and health screenings.
• Commitment to safety, professionalism, and Oak Hill policies.
• Willingness to stay beyond scheduled shifts if needed.

Responsibilities

• Assist with personal care, grooming, hygiene, and daily living tasks per plans.
• Monitor health info, document vitals when required, and maintain records.
• Administer medications per training and nursing directives.
• Transport individuals to appointments and community activities.
• Prepare meals and support during mealtimes per menus and dietary needs.
• Assist with mobility and transfers using approved equipment.
• Document daily activities and incidents in the EHR, follow regulatory requirements.
• Respond to emergencies and apply safety protocols as trained.
