Job summary

OneWell Health Care in Lorain, OH is seeking a dedicated Direct Support Professional (DSP) to provide person-centered home health support to individuals with IDD and autism. You will assist with daily living activities, health and safety, and community participation in home and community settings.

This field-based, flexible position requires documenting care, implementing ISPs, coordinating with families and care teams, and following regulatory requirements under Ohio DODD, HCBS, and HIPAA.

Qualifications

• High School Diploma or GED is required.
• CPR and First Aid certifications required.
• Experience with IDD or caregiving preferred.
• Ability to document health and behavior observations.

Responsibilities

• Provide person-centered support to individuals with IDD and autism.
• Implement ISPs and document health, behavior, and daily activities.
• Maintain required documentation and communicate with families and care teams.
• Assist with personal care, mobility, and community participation.
