Job summary

Commonwealth of Pennsylvania in Scranton is seeking a Drug and Alcohol Case Management Specialist to provide direct case management to individuals with substance use needs within the local government framework.

You will screen, assess, plan, and coordinate services with community providers, supporting determinants of health and treatment engagement in a collaborative team environment.

Qualifications

• One year as Drug and Alcohol Case Management Specialist Trainee, or a Bachelor's Degree in Chemical Dependency/Sociology/Social Welfare/Psychology/Nursing or related field with one year of direct service experience; or a Bachelor’s with 18 credits in the related social sciences and one year of direct service experience; or any equivalent combination of experience and training.

Responsibilities

• Consumer Screening and Assessment: Complete drug and alcohol screening and level of care assessments for all referred consumers.
• Care Planning: Develop service plans and link consumers with community resources to meet determinants of health and providers to implement the recommended level of care.
• Treatment Monitoring: Track treatment progress and conduct required follow-up as desired by consumer, and document in required health record systems.
• Provider Coordination: Liaison with providers to ensure coordinated services.
