Qualifications

Responsibilities

• Screen and assess drug/alcohol use and level of care for referred consumers.
• Develop service plans and link to community resources.
• Monitor treatment progress and conduct follow-ups; document in health records.
• Support county specialty and problem-solving court programs.
• Coordinate with providers to ensure integrated services.
