Job summary

Evergreen Health in Buffalo, NY is seeking a Harm Reduction Worker to support daily operations of the Drop-In Services. You will provide non-judgmental care, coordinating showers, laundry, food and beverages, and individual and group support.

Qualifications include an Associate degree in public health or related field (or two years’ experience with at‑risk populations), a valid NYS driver’s license, and excellent verbal and written communication. Travel between offices may be required.

Qualifications

• Associate degree in public health or related field, or two years of experience with at-risk populations.
• Ability to travel between office locations as needed.
• Current and valid NYS driver’s license, verified through NYS LENS system.
• Excellent verbal and written communication skills.
• Awareness of health disparities faced by underserved populations.

Responsibilities

• Provides direct patient services in the Drop-In Center.
• Greet patients, manage intake, hospitality services, and coordinate showers and laundry; create and facilitate groups.
• Document patient interactions in the EHR and collaborate with other providers and departments.
• Adheres to documentation standards and completion timeframes.
• Assists patients in identifying personal goals and follows up to address progress.
