Job summary

Cornerstone Family Healthcare in Newburgh, NY is seeking a Prevention Specialist to join our mobile van-based outreach program. You will provide syringe support, harm reduction education, counseling, and referrals to PWUD/PWID in the Mid-Hudson region, including Dutchess, Ulster, Orange, Sullivan counties and the Newburgh Hub.

The role requires an Associate’s Degree with experience in harm reduction, outreach, and community resources; bilingual English/Spanish preferred.

Qualifications

  • Experience with PWUD/PWID and harm reduction required.
  • Minimum two years of outreach experience.
  • Background in healthcare or community resources preferred.

Responsibilities

  • Observe HIPAA and confidentiality policies.
  • Maintain relationships with community resources and law enforcement.
  • Attend and provide training for opioid overdose prevention (NARCAN).
  • Provide referrals for HIV/STI/HCV testing and counseling and other services.
  • Ensure client charts are complete and compliant with standards.
  • Document outreach in AIRS database and assist with data as needed.
  • Participate in supervision and team meetings.
  • Assist in program coverage including street outreach and mobile units.
  • Attend department and all-staff meetings.
  • Follow Cornerstone policies and employee handbook.
