Job summary

Compassus is seeking a Care Partner to coordinate safe, patient-centered transitions from hospital to post-acute settings in Laguna Beach. The role involves bedside engagement, discharge planning, and collaboration with physicians, case management, and post-acute providers.

The position emphasizes reducing length of stay and readmissions while ensuring appropriate care in the right setting at the right time.

Qualifications

• Education: Associate's degree required; Bachelor's degree preferred.
• Experience: 2–3 years in healthcare preferred; no mandatory experience.
• Certifications: JV hospital partner occupational health requirements referenced.

Responsibilities

• Coordinate safe, efficient transitions of care for hospitalized patients.
• Evaluate patients for home-based and post-acute care services.
• Develop and implement transition-of-care plans with care teams.
• Educate patients and families on post-acute options and services.
• Coordinate post-acute services including home health and hospice when applicable.
• Document activities in the EMR and manage referrals.
