Job summary

Providence at Home with Compassus is seeking a Clinical Care Partner I to coordinate safe, efficient, and patient-centered transitions of care for hospitalized patients. This in-person role drives discharge planning with physicians, case management, and post-acute teams to ensure timely and appropriate home-based care.

The role emphasizes reducing length of stay and readmissions, improving outcomes, and aligning care with value-based pathways.

Qualifications

• Active LPN/LVN license or equivalent per state requirements.
• CPR certification required.
• Experience with EMR systems, preferably Epic.
• 2–3 years of experience in care coordination, discharge planning, or post-acute care preferred.

Responsibilities

• Coordinate hospital discharge planning and post-acute care arrangements.
• Evaluate referrals for home-based services and appropriate post-acute care.
• Educate patients and families on post-acute options and care expectations.
• Collaborate with physicians, case management, nursing teams, and post-acute providers.
• Participate in interdisciplinary rounds and care planning discussions.
