Job summary

Qualifications

• Required: Associate’s degree in Nursing, Health Sciences, or related field.

Responsibilities

• Evaluate patients for home-based and post-acute care services based on clinical and functional factors.
• Coordinate timely, safe discharge planning and care transitions.
• Educate patients and families on post-acute options and expectations.
• Liaise between hospital teams and post-acute providers to support referrals and placements.
