Job summary

Providence at Home with Compassus is hiring a Clinical Care Partner I to coordinate safe, efficient transitions of care for hospitalized patients. The role emphasizes bedside engagement and collaboration with clinicians to direct post-acute care decisions and services.

The position requires a Nursing-related associate degree (bachelor’s preferred) and experience in care coordination or discharge planning. Candidates should be proficient with EMR systems and able to educate patients and families

Qualifications

• Associate’s degree in Nursing, Health Sciences, or related field.
• Bachelor’s degree in nursing, Health Sciences, or related field.
• 2–3 years of experience in care coordination, discharge planning, or healthcare services preferred.
• Experience with EMR systems (e.g., Epic) and referral platforms preferred.

Responsibilities

• Evaluate patients for home-based and post-acute care services based on clinical, functional, psychosocial, and environmental factors.
• Collaborate with physicians and care teams to support appropriate level-of-care decisions.
• Coordinate hospital discharge planning and individualized transitions of care plans.
• Educate patients and families on post-acute care options and services.
• Maintain relationships with physicians, case management, nursing teams, and post-acute providers.
