Ascension at Home together with Compassus seeks a Clinical Care Partner to coordinate safe, patient-centered transitions of care for hospitalized patients at St. Thomas West Hospital.

You will evaluate patients for appropriate post-acute home-based care, support discharge planning, and collaborate with physicians, case management, families, and post-acute providers in an in-person, bedside role.

Qualifications

• Required licensure: RN, LMSW, LCSW, or LICSW with current CPR.
• Associate degree in Nursing, Health Sciences, or related field; bachelor’s degree preferred.

Responsabilités

• Coordinate safe, efficient transitions of care for hospitalized patients.
• Assess patient needs for home-based and post-acute care services.
• Develop transition-of-care plans and arrange post-acute services.
