Education required includes an Associate's degree with Bachelor's preferred; 2–3 years in healthcare is advantageous. Strong communication and teamwork are essential for success in this role.

Qualifications

• Experience: None; 2–3 years in healthcare preferred (liaison, care coordination, discharge planning).
• Strong written and verbal communication abilities.

Responsibilities

• Coordinate safe discharge planning and transitions to post-acute care.
• Evaluate patients for home-based and post-acute services based on clinical factors.
• Educate patients and families on post-acute options and expectations.
• Maintain relationships with physicians, case management, and discharge teams.
