Job summary

Qualifications

• High school graduate or equivalent; BSW-social worker preferred.
• At least one year experience working with medical patients.
• Assessment skills and sound judgment.

• Knowledge of Medicare and AHCCCS regulations/requirements.

Responsibilities

• Coordinates the integration of Discharge Planning & Swing Bed level care.
• Functions into the patient care, discharge and home planning processes with other hospital departments, external service organizations, agencies, and health care facilities.
• Provides direct care to selected patients as appropriate.
• Ensures compliance with quality patient care and regulatory standards.
