Job summary

Cobre Valley Regional Medical Center in Globe, AZ is seeking a Discharge Planning Clerk who helps coordinate discharge planning and swing bed level care. Under supervision, you will integrate discharge planning with hospital departments, external organizations, and health care facilities.

You may provide direct patient care as appropriate and ensure compliance with quality and regulatory standards, Medicare and AHCCCS rules, and effective communication with the care team.

Qualifications

• High school graduate or equivalent; BSW-social worker preferred.
• At least one year experience working with medical patients.
• Assessment skills and sound judgment.
• Effective communication.
• Knowledge of Medicare and AHCCCS regulations/requirements.

Responsibilities
