Job summary

EMPACT-SPC in Arizona seeks a Discharge Planner to coordinate case management for adults designated Seriously Mentally Ill and to assist in inpatient discharge planning.

You will participate in hospital staffing and treatment team meetings, help with Court Ordered Paperwork, and work with the Court Coordinator to ensure timely, accurate documentation. Bilingual Spanish skills are a plus, and candidates must meet state licensure and driving requirements.

Qualifications

• Bachelor's Degree in behavioral field or combination education/experience meeting BHT requirements as per AZDHS/BHS
• Master's Degree in behavioral health is preferred
• Minimum of 2 years of case management or inpatient experience in behavioral health is required
• Detail oriented with ability to multi-task in a fast-paced environment
• Fingerprint clearance card or eligibility to obtain one
• Valid AZ driver's license with no major infractions in past 3 years
• Regular access to reliable transportation
• Bi-lingual (Spanish)
• Must be 21 years of age or older

Responsibilities

• Coordinate discharge planning and case management for Adults designated Seriously Mentally Ill
• Assist treatment team in formulating discharge plans and service planning for inpatient and outpatient settings
• Participate in hospital staffing and treatment team meetings to support inpatient discharge planning
• Assist with Court Ordered Paperwork and coordinate with the Court Coordinator for timely completion
