Job summary

Qualifications

• High school education or GED required.
• Associate degree/technical training or equivalent combination preferred.
• Experience in a healthcare setting preferred.

Responsibilities

• Manages the discharge/transition process by working closely with the patient and/or family, and coordinating with the multidisciplinary team.
• Coordinates small program(s) with limited budget/impact.
• Applies substantial knowledge to perform a wide range of activities and acts as working supervisor for the team.
