Job summary

Aurora Behavioral Health System West in Glendale, Arizona is seeking an experienced Discharge Planner to join our inpatient and outpatient teams. The position focuses on quality case management and coordinating patient discharges with outpatient providers, programs, and community agencies to develop tailored aftercare plans.

Qualifications

• 2 years of case management experience.
• 2 years of behavioral health experience.
• Excellent customer service skills.
• Ability to multitask in a fast-paced environment.
• Ability to pass a drug screen (safety sensitive).
• Must obtain a fingerprint clearance card.

Responsibilities

• Collaborate with the clinical team regarding discharge plan barriers and concerns.
• Document engagements with the patient and involved parties.
• Coordinate staffing for discharge and liaise with outpatient providers and community resources.
• Manage discharge plans and communicate with external agencies.
• Schedule follow-up appointments and referrals to appropriate levels of care.
• Assist in stepping down to PHP or IOP as needed.
• Schedule family sessions and treatment team meetings to ensure safe discharge.
• Develop individualized discharge plans using treatment data and chart information.
• Identify and contact family or community resources for aftercare.
• Maintain timely, accurate documentation in the patient’s medical record.
• Maintain positive communications with internal and external customers.
• Help resolve utilization review issues.
