Qualifications

Responsibilities

• Arrange post-discharge services and referrals.
• Review patient letters and obtain required signatures.
• Communicate with patients, families and care team.
• Provide follow-up to Medicare patients prior to discharge.
• Support Case Manager with assigned tasks and documentation.
