Job summary

Southern California Hospitals is seeking a Discharge Coordinator to work under the direction of the Case Manager, coordinating post-acute care needs to safely discharge patients and ensure continuity of care. You will communicate with patients, families, physicians and facilities, handle insurance information, referrals, scheduling, and PHI handling in compliance with HIPAA.

Qualifications

• Minimum 2 years as a Patient Access Rep or Financial Counselor in a healthcare setting.
• Must demonstrate customer service skills.
• Computer literacy and proficiency.
• Excellent written and verbal communication skills.
• Ability to multitask and maintain workload pace.

Responsibilities

• Coordinates discharge plans as directed by the case manager with patients, families, physicians and providers to facilitate discharge and resources.
• Notifies receiving facility of insurance information and processes referrals post-discharge.
• Effectively schedules, reschedules, or confirms patient appointments to maximize throughput.
• Registers patients, verifies insurance coverage, and documents required information.
• Maintains PHI in compliance with HIPAA and prepares reports as needed.
