Job summary

University Behavioral Health of Denton, part of Universal Health Services, is seeking a Discharge Planner to coordinate discharge planning and ensure post-hospitalization care continuity. The role emphasizes collaboration with care teams and external professionals to create effective discharge plans for adolescents and adults in a psychiatric setting.

The position requires a Bachelor’s degree in Social Work or a related field, at least one year of hospital-related experience, and completion of

Qualifications

Bachelor's Degree from an accredited college or university, in Social Work or a clinical related mental health field preferred.
Minimum of one year experience in related disciplines with hospital experience strongly preferred.
Handle With Care training within 30 days of employment and prior to assisting in a restraining procedure. May be required to work flexible hours and overtime.

Responsibilities

Communicate with referral sources within the first 24 hours of admission.
With assistance from the Care Center, social workers, and nursing, identify all outside professionals currently working with the patient to complete discharge plans and facilitate communication to minimize duplication.
Identify needs of referral sources and/or professionals during hospitalization period.
Oversee continued communication of treatment progress, discharge plans, and time of discharge to outside parties and facilitate transfer of documents.
