Qualifications

• High school diploma or equivalent is required.
• Word processing and basic computer skills are required.
• Knowledge of medical terminology.

• Customer service skills appropriate to the job are essential.

Responsibilities

• Arrange post-discharge services for patients.
• Create and follow up electronic referrals using the Case Management system.
• Review patient choice letters with patients/families for signatures.
• Provide follow up information to Medicare patients prior to discharge.
• Communicate with patients, families and care team.
• Complete tasks assigned by the Case Manager.
• Make copies, send faxes and make phone calls.
• Complete process reviews or audits as requested.
• Perform other duties as assigned.
